Smith's fifth studio album, "Hazel Eyes," pays tribute to Cowan. People reported that the title references the designer's eye color.
Smith began writing the album's first track, "Everlasting Love", the night before the couple's first date. The song was the first song in which he wrote about "requited love".
The couple was seen together for the first time in December 2022, at the White House. On the occasion, Smith performed in celebration of the enactment of the Respect for Marriage Act by then-president Joe Biden.
They publicly came out about their relationship in May 2024. That month, the two made their first appearance on a red carpet, during the Met Gala.
Smith and Cowan also attended the 2026 Met Gala together in coordinated outfits created by the designer. Cowan wrote that the costumes were a love letter to Sam Smith and illustrator Erté.
