To receive assistance, parents or guardians must present: Identification document with photo; CPF or SUS Card of the child/adolescent; Vaccination booklet (preferably). ?The absence of a vaccination card does not prevent application. Teams can consult their history and register using only their CPF and date of birth. If the user needs it, a new passbook can be issued and delivered at the time of service. ??
