EXCLUSIVE: former employee reports problems in routine maintenance at Voepass The final report from the Center for Research and Prevention of Aeronautical Accidents (Cenipa) on the crash of Voepass flight 2283, in Vinhedo (SP), pointed out failures in the aircraft's de-icing system on the three flights prior to the accident that killed 62 people on August 9, 2024. The document also cites the absence of formal records in the technical diary on board. Some of these problems had already been reported last year by a former Voepass employee who worked in the maintenance sector. In an interview with g1, he stated that a failure in the deicing system was verbally communicated to the team in Ribeirão Preto (SP), hours before takeoff to Cascavel (PR), but was not recorded in the Technical Logbook (TLB), the aircraft's technical diary. "If the pilot reports, this aircraft would have stayed here [in Ribeirão Preto] or it would not have gone to the final destination it did. So, an accident could certainly have been avoided", stated the former employee in the interview. Cenipa's report has preventive purposes and does not seek to attribute civil or criminal liability. Former Voepass employee, in an exclusive interview with g1 Marcelo Moraes/EPTV What Cenipa pointed out In the presentation of the final report, Cenipa divided the analysis into four flights of the PS-VPB aircraft: the three prior to the accident, called “flight -3”, “flight -2” and “flight -1”, and “flight 0”, which was flight 2283, from Cascavel to Guarulhos, which crashed in Vinhedo in August 9, 2024. According to the report, data from the aircraft showed a failure in the boots of the Airframe De-Icing system, responsible for removing ice from parts of the plane's structure, on the accident flight and on the three previous flights. On “flight -3”, made from Guarulhos to Ribeirão Preto the night before the accident, Cenipa states that ice was detected with the deicing system turned off. Between this flight and the next, a routine inspection was carried out in Ribeirão Preto, but, according to Cenipa, this procedure did not include functional testing of the deicing system. On “flight -2”, from Ribeirão Preto to Guarulhos, Cenipa reported new ice detection, loss of performance, low cruising speed alert, Airframe De-Icing failure, system reset and absence of record of the occurrence in the technical diary. According to the report, this lack of registration again prevented the fault from being identified for subsequent flights. On “flight -1”, from Guarulhos to Cascavel, operated by the same crew that would later fly the accident, Cenipa pointed out new faults and warnings related to ice. The report says that despite the number of discrepancies observed, no records were found in the TLB nor formal communication between the crew and maintenance. According to Cenipa, the lack of formal registration prevented the company's technical and operational sectors from applying measures to reduce risk, such as dispatching the aircraft under specific restrictions, changing planes, route replanning or corrective maintenance. According to him, the aircraft arrived in Ribeirão Preto around 1 am and took off again at 5:32 am. During this period, according to the report, the failure was not investigated because there was no formal record in the TLB. The former employee also reported pressure to prevent aircraft from being grounded and said that the lack of registration prevented maintenance from being carried out. “The leader himself asked: ‘well, if he didn’t report it in a book, there’s no problem with the aircraft.’ That was the company’s legacy: if the commander reports it, there’s maintenance action; if he didn’t report it, they won’t waste time on anything he says,” said the former employee in the interview.
